Information We Collect

Providers Choice collects information from our website when needed to meet USDA Food Program requirements and to fulfill requests for forms, services or information. For example, the information we collect from our participating providers when Enrolling a Child Online includes things like:

  • Child's name being enrolled
  • Child's birthdate
  • Child's race and ethnicity
  • Parent/Guardian's mailing address
  • Parent/Guardian's phone number
This child information is needed to process your claims. The parent/gaurdian information is needed to verify the accuracy of your claims.

Automatic Information Collection

To help us understand and enhance our interactions with visitors to our websites, we may utilize a web analytics provider to collect demographic information using automated tools like cookies or web beacons. Demographic information collected includes:

  • Type of electonic device used to reach our website (ex computer, tablet, phone)

Providers Choice uses uses Secure Sockets Layer (SSL) encryption technology to protect your personal information during data transport. SSL protects information you submit via our website when Enrolling children online and Checking on the Status of your Claim.

Data Retention

Providers Choice is required by USDA Food Program regulations to retain any claiming records while you are participating on the Food Program with our agency, and for three years after you are no longer participating on the Food Program with our agency.

Information Sharing

Providers Choice does not sell, rent or trade your personal information to third parties. However, Providers Choice will cooperate fully with any investigation being conducted by officials responsible for the enforcement of Minnesota's child abuse and neglect statutes.
